Recent systematic reviews have found that TM for HF management reduces mortality risk and hospital readmissions and more frequent transmission of patient data increases its effectiveness. TM is the use of information technology to monitor patients at a distance (ie, at home) while empowering them to participate in their own care. Tools, such as telemonitoring (TM), have been proposed to improve clinical management and self-care of patients with HF. Heart failure (HF) is associated with poor health outcomes and high costs largely because of frequent hospitalizations.
